I have visited the doctor office atleast once, how do I login to patient portal?
You can click on sign up for patient portal link and request for access code to create a user name and password.

What if I have forgot my username or password?
If you have lost your user name or password please click on the forgot password / forgot username, follow the instructions on the screen to reset your credentials.

What if I do not remember the answers for my security questions?
Please call your Dr. Office so that they can reset your credentials and you can sign up to patient portal again.
