PrimeMD Patient Portal New user login steps

New User Login (MyChart)

PrimeMD uses MyChart as its secure patient portal.
New patients must sign up once before logging in for the first time.

Step 1: Go to the PrimeMD Patient Portal

1. Open a web browser (Chrome, Safari, Edge, etc.).

2. Navigate to the PrimeMD Patient Portal.

Step 2: Click “Sign Up” (New Users)

1. On the portal page, look for:
   “NEW USER – Don’t have a MyChart account?”

2. Select Sign Up or Request Access Code.

Step 3: Request an Access Code

You can obtain an access code using one of the options below.

​

Option A: Online Request

1. Complete the online form with:

   • First and last name

   • Date of birth

   • Email address

   • Phone number

2. Submit the form.

3. You will receive an email or text message with your access code.

Option B: Request from the Office

1. Call or visit any PrimeMD location.

2. After identity verification, our staff will provide an access code.

​

Step 4: Create Your MyChart Account

1. Click the link in your email or text message, or return to the portal page.

2. Enter your access code.

3. Create your:

   • Username

   • Password

   • Security questions

4. Confirm your email address and phone number.

Step 5: Log In to the Patient Portal

1. Return to the PrimeMD Patient Portal page.

2. Click Sign In.

3. Enter your MyChart username and password.

What You Can Do in the Patient Portal

​

Once logged in, you can:

• View lab results

• Review visit summaries

• Request and manage appointments

• Send secure messages to your provider

• Access forms and medical records

Forgot your username or password?
Select “Forgot Username” or “Forgot Password” on the login page.

​

Don’t remember your security question answers?
Please contact your PrimeMD office to reset your portal access.

Call your PrimeMD clinic for assistance.
